Maclaren USA Recalls to Repair Strollers Following Fingertip Amputations

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Maclaren Strollers

Units: About one million

Distributor: Maclaren USA, Inc., of South Norwalk, Conn.

Hazard: The stroller's hinge mechanism poses a fingertip amputation and laceration hazard to the child when the consumer is unfolding/opening the stroller.

Incidents/Injuries: The firm has received 15 reports of children placing their finger in the stroller's hinge mechanism, resulting in 12 reports of fingertip amputations in the United States.


Description: This recall involves all Maclaren single and double umbrella strollers. The word "Maclaren" is printed on the stroller. The affected models included Volo, Triumph, Quest Sport, Quest Mod, Techno XT, TechnoXLR, Twin Triumph, Twin Techno and Easy Traveller.

Sold at: Babies"R"Us, Target and other juvenile product and mass merchandise retailers nationwide from 1999 through November 2009 for between $100 and $360.

Manufactured in: China

Remedy: Consumers should immediately stop using these recalled strollers and contact Maclaren USA to receive a free repair kit.

Adventure Playsets Recall to Repair Backyard Swing Sets Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Adventure Playsets Wooden Play Sets

Units: About 275,000 in the United States and 6,800 in Canada

Distributor: Adventure Playsets, of Amarillo, Texas

Hazard: The plastic coated lumber on the horizontal ladder (monkey bar/swing beam) can weaken over time due to rotting of the whitewood (spruce, pine and fir species), resulting in a fall hazard.

Incidents/Injuries: Adventure Playsets has received more than 1,400 reports of rotting ladders involving 16 injuries that resulted in nine emergency room visits. The injuries include two arm fractures, lacerations, scrapes, and bruises to children younger than 8 years old. Most of the reported injuries occurred when the swing came out of the monkey bar/ swing beam that had rotted.

Description: This recall involves wooden play sets with swings, slides and ladders. Each set has an overhead monkey bar ladder that acts as both the monkey bar and swing beam, and an end ladder coated with cranberry or green plastic. The instruction manual has the name "Adventure Playsets" and one of the following model numbers printed on the cover.

Some Recalled Simplicity Bassinets Include Graco and "Winnie the Pooh" Brand

WASHINGTON, D.C. - - The U.S. Consumer Product Safety Commission (CPSC) is aware that some bassinets in the Simplicity recall announced August 28, 2008 include the Graco logo and "Winnie the Pooh" motif licensed by Disney Consumer Products. Consumers are urged to check their bassinets to ensure they are not using one of the recalled Simplicity models, including those with the Graco logo or "Winnie the Pooh" motif.

The Simplicity bassinets with the Graco logo were sold between 2001 and 2004. The Simplicity bassinets with the "Winnie the Pooh" motif were sold between 2002 and 2008.

Infants Strangled to Death in Simplicity Bassinets: CPSC Urges Consumers To Stop Using Simplicity 3-in-1 and 4-in-1 Convertible “Close-Sleeper” Models

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) is urging parents and caregivers to stop using convertible “close-sleeper/bedside sleeper” bassinets manufactured by Simplicity Inc., of Reading, Pa. CPSC has learned that on August 21, 2008, a 5-month-old girl from Shawnee, Kan. was strangled to death when she became entrapped between the bassinet’s metal bars. This is the second strangulation death CPSC has learned of in the co-sleeper bassinets. On September 29, 2007, a 4-month-old girl from Noel, Mo. became entrapped in the metal bars of the bassinet and died.

CPSC is issuing this safety alert because SFCA Inc., the company which purchased all of Simplicity Inc.’s assets at public auction in April 2008, has refused to cooperate with the government and recall the products. SFCA maintains that it is not responsible for products previously manufactured by Simplicity Inc.

The Simplicity 3-in-1 and 4-in-1 convertible bassinets contain metal bars spaced farther apart than 2 3/8 inches, which is the maximum distance allowed under the federal crib safety standard. The metal bars are covered by an adjustable fabric flap which is attached by velcro. The fabric is folded down when the bassinet is converted into a bed-side co-sleeping position. If the velcro is not properly re-secured when the flap is adjusted, an infant can slip through the opening and become entrapped in the metal bars and suffocate. This warning does not cover bassinets produced in recent months that have fabric permanently attached over the lower bar.

Due to the serious hazard these bassinets pose to babies, CPSC urges all consumers to share this safety warning with day care centers, consignment stores, family and friends to ensure that no child is placed to sleep in a Simplicity convertible bassinet covered by this warning. The Commission voted, using its new authorities in the Consumer Product Safety Improvement Act, to release this warning upon making a finding that the health and safety of the public require immediate notice.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) is urging retailers across the country to protect the health and safety of babies by stopping sale and recalling nearly 900,000 dangerous Simplicity bassinets. Yesterday, CPSC warned parents, caregivers and consumers to immediately stop using convertible "close-sleeper/bedside sleeper" bassinets manufactured by Simplicity Inc., of Reading, Pa., due to a serious safety risk. Name of Product: Phil & Teds e3 Single Buggy, e3 Twin Buggy and sport v1 Single Buggy Strollers (an additional 425 e3 Twin Buggy Strollers were recalled on May 5, 2006)

Units: About 44,000

Importer: Regal Lager Inc., of Kennesaw, Ga.

Hazard: The stroller’s hinge locking mechanism poses a laceration hazard to the user folding and unfolding the strollers.

Incidents/Injuries: Regal Lager has received nine reports from consumers who injured their fingers in the stroller’s hinge locking mechanism.

Description: The recalled single strollers all have metal frames with three wheels, a cloth seat and canopy. The twin strollers have a metal frame with four wheels, side-by-side cloth seats and double sun canopies. The strollers were sold in a variety of colors including red, orange, green, black, charcoal and navy. The Phil & Teds logo is located on the crotch piece of the harness.

Sold at: Baby furniture and baby products stores nationwide from August 2003 through August 2008 for between $400 and $650.

Name of Product: Children’s Hooded Sweatshirts and Jackets

Units: About 43,000

Importer: Orioxi International Corp., of Brea, Calif.

Hazard: The sweatshirts and jackets have drawstrings through the hood which pose a strangulation hazard to young children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist drawstring in upper garments, such as jackets and sweatshirts.

Incidents/Injuries: None reported.

Description: The recalled garments were sold in children’s sizes small, medium and large in the following colors: white, ivory, red, black, navy, brown, green, charcoal, blue, and pink. “BAC” is printed on the sewn-in tag on some of the garments. “Breckenridge”, “Keystone”, “Jackson Hole”, “Vail”, “Steamboat”, and “Mackinac Island” is printed on the front of the garments.

Sold at: “Shirt off My Back” retail stores located in Colorado, Wyoming, and Michigan from September 2005 through May 2008 for between $20 and $30.

Wooden Infant Toys Recalled by Habermaass Corp. Due to Choking Hazard

Name of Product: Wooden Puzzles, Infant Rattles, Pacifier Holders, and Stroller Toys

Units: About 118,000

Manufacturer: Habermaass Corp. Inc., of Skaneateles, N.Y.

Hazard: The wooden rattles, pacifier holders, and stroller toys contain small pieces including glued on mirrors and/or prisms that can detach, posing a choking hazard to infants. The head of the ladybug puzzle pieces also poses a choking hazard.

Incidents/Injuries: Habermaass Corp. has received 15 reports of incidents involving the recalled puzzles, rattles, and pacifier holders including pieces of the rattles detaching and being mouthed by young children.

Description: The puzzles, rattle toys, pacifier holders, and stroller toys are wooden and are described in the chart below. “HABA” is printed on them. Sold at: Specialty toy stores nationwide and at specialty online retailers from January 2002 through August 2008 for between $10 and $35.

Units: About 3,000

Hazard: The garments have a drawstring through the hood, posing a strangulation hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.

Incidents/Injuries: None reported.

Description: The recalled jackets and hoodies have a drawstring through the hood. They were sold in children's sizes 2 through 12. The jackets were sold in the following colors: tan, stone, bayou blue, crisp blue, white, berry, and citrus. The hoodies were sold in teal, kiwi, mandarin, aloe green, paprika, hibiscus, smokey blue, white, sherbet, cobalt, and stone.

Hazard: The cribs could fail to meet a federal safety standard for crib dimensions. When the mattress support is in the middle setting, the space between the mattress and the crib could be too wide, posing an entrapment hazard to infants.

Description: This recall involves the full size 2nd Nature Built to Grow cribs, model number 353-14-220. Model and serial numbers are located on the headboard. The crib is natural maple in color. Westminster Inc. Recalls Remote-Controlled Helicopter Toys Due to Risk of Fire

Name of Product: "Thunder Wolf" Remote Controlled Indoor Helicopters

Units: About 102,000

Importer/Distributor: Westminster Inc., of Atlanta, Ga.

Hazard: The rechargeable battery inside the helicopter can overheat. This can result in the helicopter's body melting, as well as a risk of fire or burns to consumers.

Incidents/Injuries: Westminster Inc. has received seven reports of overheating and/or melting of the helicopter, including one report of flames and two reports of minor property damage. No injuries have been reported.

Description: The "Thunder Wolf" remote-controlled indoor helicopter is made of foam and plastic, and measures about 7 inches by 3 inches. "TW0996" is printed on the tail of the helicopter and was sold in multiple accent colors. The helicopter comes with a controller and a separate charger. Only helicopters that do not have "Made in China" stamped on the underside of the helicopter are subject to the recall.

Kolcraft Recalls Play Yards After the Death of a 10-Month-Old Child

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Kolcraft Enterprises Inc., of Chicago, Ill., is voluntarily recalling about 425,000 infant play yards following the death of a child.

CPSC received a report of a 10-month old boy who strangled on the changing table's restraint strap that was hanging down into Kolcraft's "Sesame Beginnings" Travel Play Yard where the child was located.

Twelve different Kolcraft play yards are included in this recall. All of the play yards have raised changing tables with a restraint strap that forms a loop beneath the changing table, posing a strangulation hazard to a child in the play yard.

In addition to the strangulation hazard with the changing table restraint strap, one play yard also has a raised cradle that rocks back and forth. A child can roll and get trapped against the side of the cradle in the Contours 3-in-1 Play Yard. If that happens, a child can suffocate. Kolcraft has received 45 reports of children rolling to the side of the rocking cradle attachment.


The recalled play yards were sold in a variety of colors and patterns. Play yards and model numbers included in this recall are listed in a chart on CPSC's web site www.cpsc.gov. The model number is printed on a white sticker located on one of the feet of the play yard. They were manufactured in China.

The play yards were sold at retail stores nationwide from January 2001 through September 2007 for between $50 and $130.
Consumers should immediately stop using the changing tables and rocking cradle and contact the firm for instructions on receiving a free non-looped replacement strap for the changing table and a free repair kit to secure the base of the cradle. Consumers should immediately cut off and remove the existing changing table restraint strap. Consumers should always remove the raised changing table and cradle or bassinet before placing a child in a play yard.

A free repair kit for the rocking cradle will be sent to consumers separate from the replacement strap.

About 1 Million Simplicity Cribs Recalled Due To Failures Resulting in Infant Deaths

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing today a voluntary recall with Simplicity Inc., of Reading, Pa., of about 1 million cribs. The drop-side can detach from the crib, which can create a dangerous gap and lead to the entrapment and suffocation of infants. CPSC is aware of two deaths in Simplicity manufactured cribs with older style hardware, including a 9-month-old child and a 6-month-old child, where the drop-side was installed upside down. CPSC is also aware of seven infant entrapments and 55 incidents in these cribs.

CPSC is also investigating the death of a 1-year-old child in a Simplicity crib with newer style hardware, in which the drop-side was installed upside down. CPSC is warning parents and caregivers to check all Simplicity cribs to make sure the drop-side is installed right side up.

The drop-side failures result from both the hardware and crib design, which allow consumers to unintentionally install the drop-side upside down. This, in turn, can weaken the hardware and cause the drop-side to detach from the crib. When the drop-side detaches, it creates a gap in which infants can become entrapped.

CPSC is also aware of two incidents that occurred when the drop-side was correctly installed with older style hardware, though the upside down installation greatly increases the risk of failure.

The recalled Simplicity crib models include: Aspen 3 in 1, Aspen 4 in 1, Nursery-in-a-Box, Crib N Changer Combo, Chelsea and Pooh 4 in 1. The recall also involves the following Simplicity cribs that used the Graco logo: Aspen 3 in 1, Ultra 3 in 1, Ultra 4 in1, Ultra 5 in 1, Whitney and the Trio.

The recalled cribs have one of the following model numbers, which can be found on the envelope attached to the mattress support and on the label attached to the headboard: 4600, 4605, 4705, 5000, 8000, 8324, 8800, 8740, 8910, 8994, 8050, 8750, 8760, and 8996.

The cribs, which were made in China, were sold in department stores, children's stores and mass merchandisers nationwide from January 1998 through May 2007 for between $100 and $300.

As an immediate precaution, consumers should check to see if the drop-side is installed right side up. To do this, check to see that the slightly rounded rail with the decorative groove is installed at the top and the plain rail is on the bottom. Next, consumers should make sure the drop-side is securely attached to the tracks in all four corners.
If the drop-side is installed upside down or not securely attached, consumers should stop using the crib immediately. Incorrect installation can cause permanent damage to the hardware and re-installing the drop-side using the same hardware is unsafe.

Consumers should check to see if their crib contains the recalled hardware. Recalled hardware has a flexible tab at the bottom of the lower tracks and the top of the lower tracks are open. Newer hardware, which is not subject to the recall, has the flexible tab located at the top of the lower track, and a permanent stop at the bottom.

Consumers who have a crib with newer style hardware and have installed the drop-side upside down or have broken hardware, should also contact Simplicity immediately.

Hazard: The printed ink on the outer packaging of the wood case contains lead. Also, some of the black watercolor paint contains excessive levels of lead, which violates the federal lead paint standard.

Description: The recall involves the Imaginarium brand 213 Piece Wooden Coloring Case which includes crayons, pastels, colored pencils, fiber pens, paintbrush, pencil, water colors, palette, white paint, ruler and pencil sharpener in a light tan wooden carrying case. The case measures about 14 inches high by 19 inches wide.

Sold by: Toys "R" Us stores nationwide and toysrus.com from October 2006 through August 2007 for about $20.

Mattel Recalls Batman(tm) and One Piece(tm) Magnetic Action Figure Sets Due To Magnets Coming Loose

Name of Product: Batman(tm) and One Piece(tm) magnetic action figure sets

Units: About 345,000

Importer: Mattel Inc., of El Segundo, Calif.

Hazard: Small, powerful magnets inside the accessories of the toy figures can fall out and be swallowed or aspirated by young children. If more than one magnet is swallowed, they can attract inside the body and cause intestinal perforation, infection or blockage which can be fatal.

Incidents/Injuries: The firm is aware of 21 incidents where a magnet fell out of the toy figure, including a case of a 3-year-old boy who was found with a magnet in his mouth. The boy did not swallow the magnet and no injuries have been reported to Mattel and CPSC.

One Piece(tm) Triple Slash Zolo Roronoa(tm) figure with model number J4142. The 5 1/2 inch tall action figure has green hair, black pants, and has magnets in his hands which connect to magnets on various swords that the figure can hold. The model number is printed on the back of the action figure's left leg.

Sold at: Discount department stores and toy stores nationwide from June 2006 through June 2007 for about $11.

Name of Product: "Sarge" die cast toy cars

Units: About 253,000

Hazard: Surface paints on the toys could contain lead levels in excess of federal standards. Lead is toxic if ingested by young children and can cause adverse health effects.

Incidents/Injuries: None reported.

Description: The recall involves die cast "Sarge" 2 1/2 inch toy cars. The toy looks like a military jeep and measures about 2 1/2 inches long by 1 inch high by 1 inch wide. The recalled toy has the markings "7EA" and "China" on the bottom. The "Sarge" toy car is sold alone or in a package of two, and may have the product number M1253 (for single cars) and K5925 (for cars sold as a set) printed on the packaging. The cars marked "Thailand" are not included in this recall.

Sold at: Retail stores nationwide from May 2007 through August 2007 for between $7 and $20 (depending on whether they were sold individually or in sets).

Additional Reports of Magnets Detaching from Polly Pocket Play Sets Prompts Expanded Recall by Mattel

Name of Products: Various Polly Pocket dolls and accessories with magnets

Units: About 7.3 million play sets (about 2.4 million play sets were recalled on November 21, 2006)

Importer: Mattel Inc., of El Segundo, Calif.

Hazard: Small magnets inside the dolls and accessories can come loose. The magnets can be found by young children and swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforation or blockage, which can be fatal.

Incidents/Injuries: Since the previous recall announcement, Mattel has received more than 400 additional reports of magnets coming loose. CPSC was aware in the first recall announcement of 170 reports of the magnets coming out of the recalled toys. There had been three reports of serious injuries to children who swallowed more than one magnet. All three suffered intestinal perforations that required surgery.

Description: The recalled Polly Pocket play sets contain plastic dolls and accessories that have small magnets. The magnets measure 1/8 inch in diameter and are embedded in the hands and feet of some dolls, and in the plastic clothing, hairpieces and other accessories to help the pieces attach to the doll or to the doll's house. The model number is printed on the bottom of the largest pieces on some of the play sets. Products manufactured after November 1, 2006 and are currently on store shelves are not included in this recall. Contact Mattel if you cannot find a model number on your product to determine if it is part of the recall.

Sold at: Toy stores and various other retailers from May 2003 through November 2006 for between $15 and $30.

Remedy: Consumers should immediately take these recalled toys away from children and contact Mattel to receive a voucher for a replacement toy of the customer's choice, up to the value of the returned product.

The Orvis Company Recalls Children's Toys Sold with Sleeping Bags Due to Choking Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firms named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Stuffed Plush Horse/Pillows and Fairy Dolls

Units: About 220 Horses and 1,300 Fairy Dolls

Distributor: The Orvis Company, of Manchester, Vt.

Importer: Carstens Inc., of Burlington, Wash.

Hazard: The plastic button eyes on the stuffed horse and the pompom nose on the fairy doll toy can easily detach, posing a small parts choking hazard to young children.

Incidents/Injuries: None reported

Description:

Stuffed Horse: The plush brown horse/pillow was sold as an accessory with the purchase of a Horse Animal Sleeping Bag. The horse pillow has outstretched fore and back legs, a bushy black mane and tail and attaches to the sleeping bag with a hook/loop attachment on the belly of the horse. Stuffed horses with eyes directly embroidered on the plush fabric are not included in the recall.

Fairy Doll: The Fairy Doll toy was sold as an accessory with the purchase of a Unicorn Animal Sleeping Bag. The stuffed fairy doll has a sewn-on pompom nose, a pink satin dress with glitter netting as a ruffled collar, sleeves and dress apron. The doll attaches to the sleeping bag with a hook/loop attachment on the bag and between the doll's wings. The fairy doll is one of three toys that attach to the sleeping bag. Also included are a large unicorn pillow and small flat unicorn. The unicorn attachments are not included in the recall.

Sold through: The Orvis Company mail-order catalogue and the firm's Web site from September 2006 through December 2006 for about $70.

Remedy: Consumers should take these toys away from young children immediately and contact the firm to obtain a refund or replacement horse and fairy doll. Consumers can continue to use the sleeping bags while waiting to receive replacement toys.

Hazard: The window blinds have a pull cord that is looped, posing a strangulation hazard to young children.

Incidents/Injuries: None reported.

Description: This recall involves two inch Natural Faux Wood Plantation Blinds sold under the Basic Blindz(r) brand. The blinds have a yellow steel top rail, also called the head rail, a cord tilter on the left side (to tilt the slats open or closed), and a double pull cord that meets at a cord connector and ends in a single cord on the right side (to raise and lower the blinds). Stickers inside the top rail state: "Springs Window Fashion LLC, Middleton, WI 53562," "Made in China," and "Manufacturing Date Code." Blinds with manufacturing date codes JX, JY, JZ, KO, KP, KQ, KR, KS, and KT are included in the recall.

Hazard: Surface paints on the toys could contain excessive levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

Incidents/Injuries: None reported.

Description: The recalled involves various figures and toys that were manufactured between April 19, 2007 and July 6, 2007 and were sold alone or as part of sets. The model names and product numbers for the recalled toys, which are all marked with "Fisher-Price," are listed below. The toys may have a date code between 109-7LF and 187-7LF marked on the product or packaging.

Risk of Explosion and Hearing Damage Prompts Recall of Remote Control Airplanes

Name of Product: Sky Rangers Park Flyer Radio Control Airplanes

Units: About 21,000

Distributor: Estes-Cox Corp., of Penrose, Colo.

Hazard: The airplanes are launched by hand and can explode near the consumer's head, posing a risk of temporary hearing loss and injuries to eyes, face and hands.

Incidents/Injuries: Estes-Cox has received 45 reports of airplanes exploding, including 22 reports of consumers experiencing temporary ear pain or hearing being affected; five reports of minor burns to hands, faces or eyes; two reports of chest impact from debris; two reports of eye injuries; and one report of a cut hand. One consumer sought medical attention for burning eyes.

Description: This recall involves Model 4116 Sky Rangers Park Flyer radio control airplanes, which come with a black battery charger. The airplanes have a wingspan of about 14 inches, a light blue, white and orange polystyrene foam fuselage, and a copper coil on the rudder. Airplanes with a warning sticker on the fuselage near the on/off switch are not included in the recall.

New Easy-Bake Oven Recall Following Partial Finger Amputation; Consumers Urged to Return Toy Ovens

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Easy-Bake Ovens

Units: About 1 million

Manufacturer: Easy-Bake, a division of Hasbro, Inc., of Pawtucket, R.I

Hazard: Young children can insert their hands into the oven's front opening, and get their hands or fingers caught, posing entrapment and burn hazards.

Incidents/Injuries: Since the repair program announced in February, Easy-Bake has received 249 reports of children getting their hands or fingers caught in the oven's opening, including 77 reports of burns, 16 of which were reported as second and third-degree burns. Easy-Bake also received one report of a serious burn that required a partial finger amputation to a 5-year-old girl.

Description: The Easy-Bake Oven is a purple and pink plastic oven that resembles a kitchen range with four burners on top and a front-loading oven. "Easy Bake" is printed on the front of the oven. Model number 65805 and "Hasbro" are stamped into the plastic on the back of the oven. This recall includes all units with the retrofit kit. The Easy-Bake Oven is an electric toy and is not recommended for children under eight years of age. Ovens sold before May 2006 are not included in this recall.

Sold at: Toys "R" Us, Wal-Mart, Target, KB Toys and other retailers nationwide from May 2006 through July 2007 for about $25.

Description: This recall involves the Playskool Toddler "NoSpill" Sippy Cup. The 8-ounce cup is clear plastic with red trim, red and yellow handles, and a yellow tip; blue trim, blue and green handles and a green tip; or purple trim, purple and aqua handles, and an aqua tip. The serial number of the recalled cups is 382814, which appears on the back of the packaging.

Hazard: Small magnets inside the plastic sticks can fall out. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforation or blockage, which can be fatal.

Incidents/Injuries: CPSC has received one report of an eight-year-old girl who was hospitalized after swallowing loose magnets. Extensive surgery was required to remove the magnets and repair intestinal perforations.

Castles with other date codes, no date code or with a plastic rod attached with a metal screw are not included in this recall. The date code is stamped on the underside of the blue top and can be viewed through the top square shape or top "A" letter openings.

Description: The product is a green/silver infant back pack carrier made of heavy duty nylon with a lightweight steel frame. Only carriers with style number 2512 and 2592LX are included in the recall. Style numbers are printed on the metal frame of the product. The carrier has a 5-point safety harness, padded shoulder and hip straps, padded leg openings and heat support. The carrier also has a top canopy and large diaper bag which attaches to the frame. "Baby Trend" is printed on the back of the seat of the carrier. The backpack folds into its own diaper bag for storage and travel.

Incidents/Injuries: Estes-Cox has received nine reports of overheating, including one report of a plane catching fire and resulting in a minor burn injury.

Description: This recall involves Sky Squadron Model 4153 (sold at RadioShack) and Sky Rangers Model 4161 (sold at Wal-Mart) radio controlled airplanes with rechargeable lithium batteries. The airplanes have a wingspan of about 18- to 20-inches and a polystyrene foam fuselage. Model 4153 is a blue Corsair single engine airplane with a Number 15 decal and a black and blue transmitter/charger. Model 4161 is a red twin engine plane with an all-black transmitter/charger. The model numbers are on the box and instructions.

Hazard: The plastic slider on the fabric strap can break. This can cause the strap supporting the carrier to release and infants to fall out of the carrier.

Incidents/Injuries: Infantino has received 10 reports of plastic sliders breaking, including eight reports of babies falling out of the carriers. There were four reports of impact injuries where the child was taken to the emergency room. One of these children fractured her skull.

Description: This recall involves the Infantino SlingRider(tm) carriers with item numbers: 141-210; 151-210; 151-528; and 151-534. The SlingRider(tm) consists of a fabric carrier with a strap attached that is worn by the user to carry an infant up to 20 pounds. The carriers are sold in black or khaki. "Infantino" is printed on the plastic slider located on the strap. The item number is printed on a label inside the SlingRider.(tm) Products labeled "Made in Thailand" or "New 2007 Design" are not included in the recall.

Incidents/Injuries: The firm has received reports of nine incidents where magnets became loose from these puzzles. No injuries have been reported.

Description: The recalled Link-N-Lite(tm) Magnetic Light-Up Puzzle, which is a double-sided plastic magnetic puzzle for ages 3 and up, has two designs. The Disney-brand puzzle shows the Disney Princesses on the front and Ariel, from The Little Mermaid, on the back. The Marvel-brand puzzle shows "Spider-Man & Friends" on both sides. Battery Packs for Toy Vehicles Recalled by JAKKS Pacific Due to Fire Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: Battery Packs for Toy Vehicles

Units: About 245,000

Manufacturer: JAKKS Pacific Inc., of Malibu, Calif.

Hazard: The lithium-ion polymer batteries used to charge the toy vehicles can ignite while charging, posing a fire hazard.

Incidents/Injuries: JAKKS Pacific has received 33 reports of the batteries melting or catching fire while charging, including three reports of minor burns to fingers and 23 reports of property damage to flooring, seats or walls.

Description: This recall involves battery packs used with FLY WHEELS XPV toy vehicles. The battery packs were sold with a charger (item number 73904) and with the radio-controlled XPV toy (item number 73906). Item numbers are printed on the product's packaging above the barcode label. The batteries are marked "Fly Wheelz," "XPV," and "JAKKS Pacific," "Lithium-ion Polymer battery (LiPo)" followed by a list of warnings and precautions. The XPV Xtreme Performance Vehicle is not the subject of this exchange program.

Sold at: Toy stores, discount department stores and Internet retailers nationwide from August 2006 through January 2007 for about $30. Battery packs and toy vehicles packaged together sold for between $80 and $90.

Manufactured in: China

Remedy: Consumers should stop using the battery packs immediately and contact JAKKS Pacific for information on receiving a free replacement battery pack.

Hazard: The snap closures on the outerwear jackets contain excessive amounts of lead, which poses a lead poisoning hazard.

Description: This recall involves boy's outerwear jackets with a Carter's-brand logo on the front. The jackets were sold in toddler and boy's sizes. Toddler sizes range from 2T to 4T. Boy's sizes include S, M and L. The outerwear jackets were sold in charcoal, navy and red.

Sold at: Major department stores and discount department stores nationwide from October 2006 through November 2006 for up to $50.

Basix U.S.A. Recalls Children's Hooded Sweatshirts and Windbreakers with Drawstrings Due to Strangulation Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: Children's Hooded Sweatshirts and Windbreakers with Drawstrings

Units: About 20,000

Importer: Vacation Clothing, doing business as Basix U.S.A., of Lauderdale Lakes, Fla.

Hazard: The garments have a drawstring through the hood, posing a strangulation hazard to children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist by drawstrings in upper garments, such as jackets and sweatshirts.

Incidents/Injuries: None reported.

Description: This recall involves children's hooded sweatshirts and windbreakers sizes S to XL. Hooded sweatshirts included in the recall were sold in pink, white, light blue, yellow, navy and gray. The nylon windbreakers were sold in royal blue, olive, pink, light blue and navy. A tag sewn inside of the garment reads, "Basix USA." Style number PK215, PK218, KPF265, KPF275, or KH185 is printed on the garment's hang tag.

Sold at: Discount department stores nationwide from May 2003 through December 2006 for between $20 and $50.

Manufactured in: China

Remedy: Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard.

Toddler Pants Sets Sold Exclusively at Meijer Stores Recalled for Choking Hazard

Name of product: Toddler Pants Sets

Units: About 1,800

Manufacturer: G & W Industries Inc., of New York, N.Y.

Hazard: The zipper pull on the jacket can detach, posing a choking hazard to young children.

Incidents/Injuries: G & W Industries has received one report of a zipper pull detaching into the hands of a toddler. No injuries have been reported.

Description: This recall involves "Little Lass" toddler pants sets sold with a velour jacket and pants, and a tee-shirt. The sets were sold in red and black, and have the words, "Snow Angel" printed on the front of the jacket. Sizes included in the recall are 2T, 3T and 4T.

Sold at: Meijer Stores nationwide from October 2006 through December 2006 for about $20.

Manufactured in: Philippines

Remedy: Consumers should immediately return the recalled pants sets to the nearest Meijer Store for a full refund.

Easy-Bake Ovens Recalled for Repair Due to Entrapment and Burn Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should immediately take the recalled oven away from children under 8 years of age and contact the firm for a retrofit with warning.

Name of product: Easy-Bake Ovens

Units: About 985,000

Manufacturer: Easy-Bake, a division of Hasbro, Inc., of Pawtucket, R.I.

Hazard: Young children can insert their hands into the oven's opening and get their hands or fingers caught, posing an entrapment and burn hazard.

Incidents/Injuries: Easy-Bake has received 29 reports of children getting their hands or fingers caught in the oven's opening, including five reports of burns.

Description: The Easy-Bake Oven is a purple and pink plastic oven that resembles a kitchen range with four burners on top and a front-loading oven. "Easy Bake" is printed on the front of the oven. Model number 65805 and "Hasbro" are stamped into the plastic on the back of the oven. The Easy Bake Oven is an electric toy and is not recommended for children under eight years of age. Ovens sold before May 2006 are not included in this recall.

Sold at: Toys R Us, Wal-Mart, Target, KB Toys and other retailers nationwide from May 2006 through February 2007 for about $25.

Manufactured in: China

Remedy: Consumers should contact Easy-Bake between 8:30 a.m. and 4:30 p.m. ET 7 days a week to receive a free retrofit kit with consumer warning. Caregivers should keep the Easy Bake Oven away from children under eight years of age.
